As a way of thanking tj7 and Krokodyle and everyone else on this forum I made a new cover with the correct date. I was also able to incorporate the photo that I took in 1983.

The high rez version should be the exact size for most DVD covers -- 10.75" X 7.25"
The fold in the center is a half inch. Measure 5 1/8" (5.1250) from the left side to find the first fold line; then measure a half inch from there to find the second fold line. You can also just find another already made DVD cover and follow their folds.

I was able to print out a version on 8.5 X 11 paper and it worked fine.

I can also make another version tonight with crop marks to make it easier.

Thanks for the nice comments! And thanks to everyone on this board for being so great about sharing. I knew there was a version of this show floating out there somewhere but I didn't know I'd actually get to see it!

I watched it, too. Brought back A LOT of memories!
The band was in fine form that night.
It's a bit surreal because most of it is shot on the stage. Only toward the end do they show the audience; otherwise it's almost like they're performing on an empty soundstage. This is the same footage that I watched on the large video screens as a young teen in 1983 at the coliseum. (I was in the nosebleed section.)
Great performance and great quality.
